Rural Health Center
Description of Service Provided:
This program is an integration/collaboration between Behavioral Health and Medical Health Services in a Primary Care setting. This program provides Outpatient Mental Health counseling to children, adults and families. In this setting, it provides an opportunity for consultation with the individual’s primary care provider as needed.
Geographic Area of Service:
The program is located at 5 Health Care Centers.
There are 3 locations in Potter County: Port Allegany, Coudersport, and Ulysses, PA.
Two locations are in Emporium (Cameron County).
This service is provided at Federally Qualified Health Care Centers. For those who have no insurance coverage, fees are based on a sliding income scale. Individuals who have health insurance benefits are also accepted.
How to Access Service:
Individuals can self refer by calling one of the offices to schedule an appointment. When you call, please provide information regarding the referral reason. Issues addressed in therapy vary from depression, anxiety, and work related concerns, to children with behavior problems, attention difficulties, and/or adjustment issues. We also provide referrals to other services and/or programs if we find another service might be more beneficial.
Locations and Hours of Operation:
Northern Potter Health Center
Mondays 8 a.m. to 12 noon.
Tuesdays 8 a.m. to 5 p.m. and Fridays 8:30 a.m. to 10:30 a.m.
Wednesdays and Thursdays from 8 a.m. to 5 p.m. and Fridays from 11:30a.m. to 4 p.m.
